BCAM Coaches Clinic Registration Form

Special: Pay for 3, get 4th FREE (free person must be a BCAM member)

Membership Requirement: If you mark that you are a “member” to get the $95 Member Rate, you MUST become a paid BCAM member no later than September 23rd or you will be asked to pay the balance of the “Non-Member Rate” when you check-in at the Clinic OR pay for your membership.

A 3% fee will be added to your total to help recoup some of our credit card costs

Receipt: After completing this form and paying online, you will receive a payment receipt via email from Authorize.net

Banquet: The cost to attend the Hall of Fame Banquet at 3:00 PM, SUNDAY, October 1st will be $30 in addtion to your clinic fee unless you are receiving an award at the banquet.
